Angel Food Ministries Order Form

Those wishing to place an order with Angel Food through Avenue Church must use the order form available on this website or through the Avenue Church office.  No orders will be received over the phone or through e-mail.  Payment must be made when placing the order.  Please be sure to give attention to the deadlines on the forms to place your orders.  Orders mail be mailed to the Avenue Church office as long as they arrive by the deadline, or, dropped off at the church on the days listed on the form.
